The blacklist is applied by HUB2, not by the providers, although providers may have similar mechanisms of their own.
HUB2 can blacklist an MSISDN (phone number) for transfers. A transfer to a blacklisted MSISDN fails with the failure cause blacklisted_msisdn, before it is sent to the provider. HUB2 keeps a separate blacklist for payments: a payment from a number blacklisted for payments fails with the same cause. An MSISDN is blacklisted for transfers when transfers to that number fail repeatedly within a short time.

Unblocking

A blacklisted MSISDN is automatically unblocked after a few days. To request an earlier unblocking, contact HUB2 support.
